Home Care Services

Our services are available from a minimal care need of 2 hours, to a 4 hour option up to 24 hours a day 7 days a week, holiday and weekends included. A Care Coordinator is also on call 24 hours a day 7 days a week to answer any questions you may have. We are unique in that the owner of our company is a Registered Nurse which offers additional medical insight to each of our clients needs.

We offer a wide range of In-Home Services so that we can provide our seniors and other adult clients with the right amount and type of help to support independent living in their home environment.

In-Home Companionship and Care Services

Conversation and Companionship
Southwest Health Care Services provide clients companionship, engaging in daily conversation, social contact to enhance their daily living and mental stimulation.

Meal Preparation
Southwest Health Care Services prepares hot nutritious meals, allowing client to help with preparation in accordance with their mobility and ability. Many seniors tend to get a quick meal not always a well balanced nutritious meal, which is necessary for a healthy lifestyle. We encourage our clients to have adequate fluid intake for good hydration, unless there is a fluid restriction ordered by a physician.

Light Housekeeping
Southwest Health Care Services provides light housekeeping as part of Activities of Daily Living when we are servicing the client in their home. Vacuuming, dusting, sweeping, and tidying up after personal care services such as bathing, and meal preparation are completed. Keeping a clean, neat environment usually makes a client feel better and creates a healthier home. Southwest Health Care Services does not allow the caregiver to use a ladder to obtain items for clients and or for cleaning purposes. We do not provide outdoor cleaning or maintenance, and do suggest that families hire a license and bonded cleaning service if a major cleaning is needed.

Laundry
Linen changes, and personal laundry including bath towels and items used in the daily client care needs are washed, dryed and put away as time allows and ironed if needed. We involve the client on their preference.

Grocery Shopping/Errands
Southwest Health Care Services shop for groceries at the stores of the client's preference and run errands, such as picking up prescriptions, and other misc. tasks. Our office can also assist clients with home grocery delivery services. Our office places the client's order online for delivery by a local grocer. The order is placed with delivery the following day directly to the home. The store that provides this service do have a nominal fee for delivery.

Incidental Transportation
Southwest Health Care Services provides transportation for clients, such as medical appointments, beauty salons, barber shops, shopping etc. Getting seniors out enhances their mind, spirit, and many times motivates them to really enjoy a day.

Medication Reminders & Case Management
At Southwest Health Care Services we offer Case Management and Medication Management with a minimum of 20 hours a week of care from our company. This service is coordinated with the client's physician and other professionals to help coordinate the best care plan for each persons care needs. We can remind a client to take their prescribed medication and monitor compliance. We do offer medication management and due to the unique fact that we are owned by nurse's and have a nurse on staff we are able to offer medication management with 20 hours a week minimum of service, free of charge. We will coordinate with written permission from the client and their physician the medication management and coordination between the pharmacy and physician.

Live-In Services
We provide Live-In companion care to clients who need fulltime monitoring; this is a bonded, insured caregiver who lives in the client's home to provide in-home care on a daily basis. State law does mandate room and board to be included and appropriate sleep patterns for the live-in. This service gives family members peace of mind, preventing gaps in coverage for individuals who may be at risk when unattended, such as, persons with dementia, Alzheimer's or other memory impairment conditions who need to be observed. A personal care plan is tailored for each client's individual needs.

24 Hour Care
Clients' needs vary and many times they need continuous care when they are at risk if left unattended. This can vary from 4 hours of service to 24 hours with shift work coordinated by Southwest Health Care Services. Many times this is needed for Hospice clients also.

Respite Care or Relief for Families
Respite Care is a wonderful way to give relief to a family member who needs to run errands, go shopping, or get some needed rest. It also helps them to still engage in the activities they have always been apart of, while still fulfilling the care they want to provide to their family member.

Personal Care Services

Activities of Daily Living
ADL's as they are referred to encompass Bathing, Dressing, Grooming, Hygiene, Toileting, Incontinence Care, and Feeding. These are things we do routinely each day. Southwest Health Care Services compassionately cares for seniors and other adults, helping them maintain dignity with these needs. Loss of independence is difficult as we get older. Willingness to accept, these services can enhance their mental as well as physical well being. Inclusive of this is preparing a well balance meal, such as preparing the appropriate meal for diabetic persons as well as a prescribed diet such as low sodium, lactose intolerance and many others.

Transferring and Positioning
Proper transferring and positioning in and out of beds and chairs is very important. Proper posture, eliminates pressure areas of the skin, and helps with mobility. In some cases, medical equipment can be ordered to enhance the assistance for the patient and caregiver.

Products that Support In Home Care
There are thousands of products that help support In-Home Care and many assistive devices and modifications. Some examples are listed below.

  • Lift chairs
  • Bedside commodes
  • Dressing aids that help seniors put on socks, ted hose and clothes
  • Eating utensils aids that can be held by those with arthritis and other disabling abilities
  • Talking devices for those with vision problems
  • Magnification products for low vision
  • Cooking aids
  • Mobility aids
  • Grab bars for toilets, tubs and showers areas
  • Remove unnecessary throw rugs and fasten down rugs or floor runners to prevent slipping
  • Move furniture to create clear walking paths
  • Keeping objects off the floor such as oxygen lines and secure cords to the wall to prevent falls
  • And many more.


With more persons choosing to "age in place" rather than moving to an alternate retirement setting, many are finding they need to modify their homes to make them safer and easier to get around.

Home modifications help senior and older adults maintain a quality of life by preventing injuries and loss of independence. Simple changes without cost can be adapted in most home settings.
